Lumbar disc herniation is one of the most common spinal conditions. It occurs when the soft inner material of an intervertebral disc in the lower back pushes through a tear in the outer layer, compressing spinal nerves. The most common levels are L4-L5 and L5-S1. When disc material compresses the sciatic nerve, it causes sciatica — sharp, shooting pain that travels from the buttock down the leg, often below the knee. Most patients improve with time and non-surgical care (physical therapy, medication, injections). However, when symptoms persist despite conservative treatment, or when there is significant weakness or progressive neurological deficit, surgical intervention may be necessary. Dr. Basques is one of only a handful of surgeons in New England offering endoscopic microdiscectomy — a same-day procedure using a camera the size of a ballpoint pen.

Diagnosis
Diagnosis begins with a detailed neurological exam testing strength, sensation, reflexes, and straight-leg-raise testing. MRI of the lumbar spine is the gold-standard imaging study confirming the level and severity of herniation. X-rays evaluate alignment and instability. In complex cases, CT or EMG/nerve conduction studies may supplement the workup.
